How We Use Your Health Information
When you receive services from us, we may use the health information you provide for assessments, billing for services, and conducting our normal business known as behavioral, mental, disability, and emotional health referral services and informational resources. Additionally, and with your consent, we may pass your information to our service provider partners, such as treatment programs, schools, and mental health and therapy professionals. Finally, we will disclose any information you may provide to comply with any court order or legal request from a law enforcement agency.
Examples of how we use your information include:
Intake and Assessment Information – We keep records of the information you provide to us and use that information to prescreen and determine which of our service partners or which professionals, if any, we would refer you to. With your consent, we will pass the information you give us on to them so they can contact and work with you to determine if they are a good fit for the solution and quality of care and treatment you are seeking.
Payment – We keep billing records that include payment information and documentation of the services provided to you. Your information may be used to obtain payment from you, your insurance company, or other third parties. We may also contact your insurance company to verify coverage for your care or to notify them of upcoming services that may need prior notice or approval.
Behavioral Health Care Services Operations – We use health information to improve the quality of our services, train staff, provide customer service, manage costs, conduct required business duties, and make plans to better serve the parents, families, and the youth behavioral and mental health industry. For example, we may use your health information to evaluate the quality of treatment and services provided by our own personnel or by service partners.
Other Services We Provide

Sharing Your Health Information
There are limited situations when we are permitted or required to disclose health information without your authorization.
These situations include:
▪ For public health purposes such as reporting communicable diseases, work-related illnesses, or other diseases and injuries permitted by law; reporting births and deaths; and reporting reactions to drugs and problems with medical devices;
▪ To protect victims of abuse, neglect, or domestic violence;

▪ For lawsuits and similar proceedings;

Your Individual Rights
You have the right to:

▪ * Request to inspect and copy your health information, including medical and billing records. Fees may apply. Under limited circumstances, we may deny you access to a portion of your health information and you may request a review of the denial;
▪ * Request corrections or additions to your health information;
▪ * Request an accounting of certain disclosures of your health information made by us. The accounting does not include disclosures made for treatment, payment, and health care operations and some disclosures required by law. Your request must state the period of time desired for the accounting, which must be within the six years prior to your request and exclude dates prior to April 14, 2003. The first accounting is free but a fee will apply if more than one request is made in a 12-month period; and

What Our Privacy Policy Covers With Respect to Your Personal Information Other Than Medical or Health Information
What personally identifiable information is being collected?
When you visit the Teen 180 website, in order for us to provide you with the products or services you have requested, you may be asked to provide us with personally identifiable information. For example, when you order a product, we may ask you to become a registered user of our site, during which you will be asked for your shipping and billing addresses, your e-mail address, and your credit card number or other payment-related information. If you request one of our newsletters or send an inquiry to one of our customer service representatives, we will also store a record of your request along with your e-mail address. How is the information being used?

With whom do we share your information?
In order to operate our websites and in order to provide you with the products and services you have requested, we occasionally need to share information with third parties. What are cookies and how do we use them?
Like many online companies, Teen 180 utilizes "cookies” to improve service to our customers. A cookie is a block of text placed in a file on your computer’s hard drive by a website you may be visiting. A cookie is an alphanumeric file that transfers to the user's computer during a session to help improve the Internet experience. These types of cookies cannot identify individual users specifically, such as by name or address, unless the cookie data is attached to personally identifiable information such as online registration. The uses of cookies by Teen 180 includes: (1) keeping track of the pages previously selected during the session, in order to enable "Back to Previous Section" capability; (2) enabling general usability statistics that can be used to make our websites better; and (3) in situations where users proactively identify themselves (e.g., with a user name and password), cookies help serve up personalized functionality without the need for users to continually re-identify themselves during their session. Cookies are randomly generated, are used only for one session and are then destroyed, and are incapable of identifying the user. Teen 180 does not control the use of cookies used or linked by contracted or third party services.
We may also use session cookies, which are pieces of identification between your browser and our server. These cookies help us do things like keep track of the items in a user's shopping cart, and other information. Shopping cart cookies are saved for a designated period of time in case a user’s session expires or she returns to complete a purchase within that time frame. For the convenience of our users, we may also place a cookie on a user’s computer when she logs in to talk forums. This gives the advantage of not having to log back in when moving between the talk forums and the rest of our sites.
What choices are available to you regarding the collection, use and distribution of the information and how you can correct any inaccuracies in information?

What kind of security procedures are in place to protect the loss, misuse or alteration of information under our control?
Teen 180 values your trust and is committed to doing everything possible to make your experience with us, including online ordering or other services, a secure process. We will share your information only with those partners and affiliates for who you give your express permission.
Secure Ordering Process
Any credit card information you enter on our sites is strongly encrypted by industry-standard protocol, SSL (Secure Sockets Layer). Teen 180 websites use SSL to communicate with secure browsers like Internet Explorer. These browsers encrypt your credit card and personal information in a way that makes it extremely difficult for anyone to intercept and misuse while being transmitted between your computer and our server.
Once you have completed your order online or have begun using or receiving other services from us, our security procedures do not end. In accordance with federal law, we do not knowingly collect personally identifiable information from children under age 13 without the verifiable permission of a parent or guardian. If you are under age 13, you are welcome to browse our websites. However you are not permitted to use any of the features of our sites that require the collection of any personally identifiable information. If we determine that we have erroneously collected a child's information without a parent’s consent, we will immediately undertake to delete that data from our records.
